I hereby release, indemnify and hold harmless Bridge of Hope Ministries, the organizers, sponsors and supervisors of all activities from any liability in connection with an injury caused by negligence. I likewise hold harmless from liability any person transporting me to or from Bridge of Hope Ministries activity. I understand that after volunteering four times within a calendar year I will be subject to a background check and required to complete volunteer compliance paperwork. In addition, Bridge of Hope Ministries has permission to utilize any photograph or video taken of me for publicity purposes.
